When attending the walk in centre for the first time you will be asked to complete a short registration form. Please help us to help you by completing all of the requested information. You will only have to do this the first time you attend as this information will be kept on our secure server for any subsequent visits that you may make to the walk in centre. We will however ask, each time you attend for permission to share the details of that visit with your own GP. This is best practice for keeping your medical records up to date. We will of course respect your wishes if you prefer confidentiality.
